If you have Insurance

As a courtesy to you:
•  We will bill your insurance company on your behalf if you provide complete and accurate information in a timely manner.
•  All co-insurance co-pays or deductibles are due at the time of your service.
•  Colorado law prevents us from offering discounts on co-insurance, co-pays and deductibles.
•  You will receive statements informing you of the status of your claim. We encourage you to stay involved with your insurance company to ensure your claim is paid. Any amount not paid timely by your insurance company may become your responsibility.
•  For the patient portion not covered by insurance, you will be asked to pay in full at the time of service.

If you do not have Insurance

•  You will receive a 10% Senior Citizen discount (55 years and older).
•  For emergency and urgent care you will be asked to pay the estimated amount due at the time of services.
•  For elective care you will be asked to pay the full amount. If payment is made prior to receiving services, you will receive a 10% discount.

Financial Assistance

•  Interest free payment plans through Care Credit are available to qualified individuals.
•  A savings plan where by the treatment plan costs are divided into four payments. These payment are made and saved in advance. Once the sum has been set aside, treatment can begin.
•  All major Credit cards, checks, and cash are accepted.
